History

St. Elizabeth Campus was established by the Sisters of Charity of Our Lady, Mother of the Church. The Sisters of Charity devote themselves to caring for the sick, poor, and aging, assisting with education, and much more. The congregation’s leaders, Monsignor John Zwijsen and Mother Marie Alma Lafond, founded the Sisters of Charity as a response to the needs of God’s people.

Saint Elizabeth Home (Skilled Nursing)

Saint Elizabeth Home was officially opened on April 15, 1967. The original home is now used as a convent for the Sisters of Charity of our Lady, Mother of the Church. Sister Mary Denise was the former Director of Nursing and Administrator of the Home, and an instrumental player in getting the Home where it is today. Today, our skilled nursing facility continues to offer quality healthcare if a spirit of compassion, love, and service according to the Gospel of Jesus Christ.

Saint Elizabeth Home is a skilled nursing residence small enough to truly feel like family, yet large enough to offer a wide range of services. We provide personalized care for older individuals with medical needs requiring more assistance that can be given in one's own home.

Saint Elizabeth Home has nurses and additional caregivers on staff 24 hours a day so that we can provide exceptional and immediate care at any time. Therapy services, phlebotomy, and x-rays are just a few of our many one-site medical capabilities.

St. Elizabeth Manor (Assisted Living)

St. Elizabeth Manor has been touching lives in Rock County and the surrounding areas since April of 2009.  We are a not-for-profit, community-based residential facility (CBRF) serving persons of all faiths.  The Manor is equipped with many features that provide a welcoming environment.

St. Elizabeth Manor is the perfect solution for seniors who wish to live in a home-like atmosphere, and remain as independent as possible, but need assistance with daily activities.

Our 24/7 care staff is available for all of your needs.   

Individualized Care Plans:  At St. Elizabeth Manor we emphasize respect for each individual’s wishes from the moment each resident arrives until the end of his or her life. We initially meet with each resident and their family prior to move-in to assess the level of care needed. Reassessments are scheduled regularly and on an as-needed basis, thereafter, to ensure that proper care is being provided.

Apartment Availability:  Three apartment layouts, all designed for convenience and comfort, are available to select from. Each apartment includes a private bath, emergency response system, and private climate control. Pets are welcome provided they pass a screening to ensure safety for all residents. Furnished rooms can be provided upon request.

Standard Apartment: 247 square feet

Mid-size Apartment: 354 square feet including a kitchenette

Double Apartment: 494 square feet 

Suite Apartment: 731 square feet including a kitchenette, living room, bedroom, and walk-in closet.
